Individuals with developmental disabilities who live in their own home or with family may need ongoing assistance coordinating services, participating in the community, or building skills for greater involvement in daily decisions. Personal Supports and Supported Living Services can be used when an individual and their existing support network need assistance organizing services and connecting with community resources while the person remains in their current living arrangement. In Anne Arundel County and other communities served by SHSG, support can involve working alongside the individual’s existing network to coordinate the services, materials, and assistance identified as needed.
How It Works
Personal Supports and Supported Living Services may include the following:
Community integration and inclusion of individuals
Individual-directed, outcome-oriented planning based on the individual’s circumstances and goals
Supporting individuals and families in making choices
Assistance coordinating needed services and materials or supplies
Training for family members or guardians on how to support the individual’s needs
Twenty-four-hour emergency assistance
Communication and social skills training for individuals
What to Expect
Support is provided within the individual’s existing home and family or community setting, with attention to the services and resources identified through individual-directed planning. SHSG can work with the individual’s existing support network to coordinate needed services and materials while providing training or skill development as appropriate. The individual’s circumstances and choices remain central to how support is organized, with services addressing identified needs rather than requiring a change in living arrangement.
